It is the client’s responsibility to disclose any allergies they have:
I agree to release the makeup artist from liability for any skin complications due to allergic reactions. I understand that I will be in direct contact with various cosmetics and products topically applied. I am aware of the specific chemicals and / or cosmetics of which I am allergic or have a sensitivity to & have notified the makeup artist.
All brushes and makeup products are kept sanitary and are sanitized between every makeup application.
Any skin condition should be reported by the client to the makeup artist prior to application and, if need be, a sample test of makeup may be performed on the skin to test for reaction.

Any photography/video taken by Maddison Avenue on the day is allowed to be used on social media platforms as advertisement. I understand that I will not be compensated for the use of these videos and images.

If the artist feels that the client has any open facial wounds or a skin condition that could be infected or unsanitary, they have the right to refuse service to that client.
If the Makeup Artist feels uncomfortable, threatened, or verbally abused in any way, they will have the right to leave the premises without completing services. No refund will be issued in the event of this happening.

I understand that Maddison Avenue keeps her kit sanitary and in doing so I am unable to touch or handle any of her products. If I destroy or contaminate any products or materials purchased by Maddison Avenue I will be billed for the cost of the replacement of said products and materials.

I agree to the fees outlined on the attached invoice. I am aware that at the time of booking for any service provided by Maddison Avenue I am to pay a non-refundable 30% deposit to secure my appointment time for any service rendered by Maddison Avenue. I am also aware that the remaining amount, outlined in the invoice, will be paid on the date of service.
